A survey for parents and carers by the London Borough of Sutton

If you have a child or children aged birth to 14 years (or 18 if your child has a special educational need or disability) then please complete this questionnaire by Friday 19th January 2018.

The London Borough of Sutton seeks to collect the views of families with children under the age of 14 or 18 if the child has a disability, on the availability of childcare within the borough. The purpose of this survey will help the London Borough of Sutton to ensure that families have access to flexible childcare that meet their childcare needs.

For the purpose of this survey ‘childcare’ refers to paid for provision, where an adult provides supervised care for a child. This could or could not be in a Ofsted registered provision, for example a childminder, day nursery, pre-school, after school club or holiday scheme.

This questionnaire is anonymous and is being administered by the London Borough of Sutton’s Family Information Service.  Results will be presented to the London Borough of Sutton but no-one will be able to identify you from your responses to these questions.
