This survey is to help us understand the experiences of residents who have been subject to Safeguarding Enquiries under Section 42 of the Care Act 2014.

You do not have to give your name or contact details unless you want to.

Your responses will be collected and reviewed by a team that is separate from Adult Social Care. The findings will be shared with Adult Social Care managers and leaders.
